Quest Behavioral Health IN and KY
Medicaid and Medicare insurances are not accepted.

Private Pay Rates
No Surprise Act / Good Faith Estimate
Effective, January 1, 2022, the No Surprises Act (NSA), protects clients from unexpected balance billing for emergency and certain non-emergency services. It mandates that providers give uninsured or self-pay patients a "Good Faith Estimate" (GFE) of costs before services.
